A bit about the role

The After-Hours Outreach Service (AHOS) team utilises an assertive outreach model to assist adults with complex health needs who are primarily vulnerable, sleeping rough, living in insecure accommodation or accessing services in Melbourne. 

You will start and end your shifts at The Living Room but will be working across Melbourne CBD 

Part Time role that requires 11 hours per week (with shifts between 4pm – 11pm on the weekend) 

If you worked here this past month here are some things you might have been involved in: 

  • Providing comprehensive nursing assessment and care to assist clients who either experiencing or at risk of homelessness 
  • Providing clinical support and expertise to members of your team  
  • Attend joint care meetings for clients  
  • Attend team meetings and 1:1’s with your manager  
  • Conducting assertive clinical outreach to engage and assess people sleeping rough in Melbourne’s CBD 
  • Delivering nursing interventions to people sleeping rough in Melbourne’s CBD 
  • Ensuring high-quality care coordination processes for clients  
  • Engage in professional development  
  • Linking clients into the Living Room services and programs 
  • Triaging and assessing new clients 
  • Conducting clinical assessments and screenings of clients  
  • Contributing to a model of integrated care with the existing service system to ensure enhanced service coordination and collaboration 
  • Liaising with community and allied health services to ensure care planning and appropriate referrals are in place  
  • Actively participate in relevant networks and external meetings

The Living Room is a primary health service, the team works with people aged 18+ to provide free healthcare and support to improve the physical, mental and social well-being of individuals who are, or at risk of experiencing homelessness. 

The only service of its kind in Melbourne, focuses on the social determinants of health to look beyond disease or illness, to see a person's overall situation. Our multi-disciplinary team works with individuals to identify goals and focus on harm minimisation, self-determination, and empowerment. 

We are a non-judgemental service with a strong focus on respect, client centred care and harm prevention. We work with clients on a personalised, tailored basis to address highly complex issues faced by people experiencing multiple and complex needs. 

Youth Projects is an independent, registered charity providing front line support to young people and individuals experiencing disadvantage, unemployment, homelessness, alcohol and other drug issues. Salary 

We’re a non-profit organisation that fills from a full cup but not a full pocket. In order to attract strong talent, we pay competitively and remunerate based on your skills and experience and are determined to make this the best job you'll ever have. This position is at an hourly rate of $40.
